Role Overview: As a Housing Management Specialist, you will play a critical role in supporting the Medical Management/Health Services team by ensuring that housing-related needs are effectively addressed for our members. Your expertise will help improve health outcomes by coordinating housing resources and services that support overall member wellness. What You'll Do: - You will assess and manage housing needs for members to support their health and recovery. - You will collaborate with healthcare providers, social workers, and community resources to coordinate housing solutions. - You will monitor and evaluate housing programs to ensure compliance with regulatory standards and organizational policies. - You will maintain accurate records and documentation related to housing services and member interactions. - You will provide education and guidance to members regarding housing options and resources. - You will participate in interdisciplinary team meetings to develop comprehensive care plans. - You will identify barriers to housing and work proactively to resolve issues impacting member health. - You will contribute to continuous improvement initiatives within the housing management program. What You Bring: - Minimum of 3 years of experience in housing management, social services, or a related field. - Strong knowledge of housing regulations, community resources, and healthcare integration. - Excellent communication and interpersonal skills to effectively engage with diverse stakeholders. - Ability to manage multiple cases simultaneously with strong organizational skills. - Proficiency in data management and documentation tools. - A compassionate and member-focused approach to service delivery. Bonus Points If You Have: - Experience working within a healthcare or managed care environment. - Familiarity with Medicaid and Medicare housing support programs. - Certification in case management or social work. - Bilingual skills or experience working with diverse populations.
