POSITION SUMMARY HHA/PCA caregivers provide hands-on personal care, assistance with activities of daily living (ADLs), companionship, and support services to clients in the home. Caregivers ensure each client receives safe, respectful, and person-centered care according to the plan of care developed by the RN. RESPONSIBILITIESPersonal Care & ADLs • Assist with bathing, grooming, dressing, and toileting • Provide safe ambulation and transfers (using proper body mechanics) • Support with meal preparation, feeding, and hydration • Assist with incontinence care and hygiene • Monitor skin condition and report changes to the RN or office immediately Household Support • Light housekeeping duties (laundry, dishes, linens) • Maintain a clean, safe home environment • Prepare simple meals aligned with dietary restrictions Client Support & Companionship • Provide emotional support, social engagement, and companionship • Encourage independence and mobility • Accompany client to medical appointments when approved Communication & Documentation • Document visits accurately and submit timesheets/electronic visit verification (EVV) through HHAeXchange • Communicate any changes in condition, incidents, or concerns immediately to coordination or RN • Follow all agency protocols, HIPAA, and reporting guidelines Professional Conduct • Maintain client dignity and privacy • Follow infection control procedures • Adhere to care plan, schedule, and agency policies • Arrive to shifts on time and notify the office of any changes or emergencies QUALIFICATIONSRequired: • Valid HHA or PCA certificate from a NYS-approved training program • Physical and PPD/QuantiFERON (recent within 12 months) • Immunizations per DOH requirements (MMR, Varicella, Flu, COVID—if required) • Two professional references • Valid ID and work authorization Preferred: • Experience working with elderly, disabled, or chronically ill individuals • Bilingual (English/Spanish/Creole/Russian/Urdu/Bengali/French) a plus • Experience with dementia/Alzheimer’s care, stroke care, or mobility support SKILLS & COMPETENCIES • Compassionate and patient-centered • Strong communication and observation skills • Reliable, punctual, and professional • Ability to follow instructions and work independently • Respectful and culturally sensitive BENEFITS (Agency-Provided) • Competitive wage rates & weekly pay • Wage parity & union benefits • Paid orientation and in-service training • Flexible schedules (live-in, hourly, weekend cases) • Opportunities for additional hours and long-term cases • Supportive office team and 24/7 on-call assistance • PCA upgrade training available (for those transitioning from CDPAP/PPL) Job Types: Full-time, Part-time, Temporary Pay: From $19.62 per hour Benefits: • Flexible schedule • Health insurance Work Location: On the road
