TITLE: Care Management Coordinator SAGE is the world’s largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual and transgender (LGBTQ+) older adults. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources for LGBTQ+ older adults and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people, and provides cultural competency training for aging providers and LGBTQ+ organizations, largely through its National Resource Center on LGBTQ+ Aging and its growing social enterprise SAGECare. Summary: The Care Management Coordinator provides essential administrative and clinical support within the Care Management department, with responsibilities split between program operations support and direct service delivery to participants. Responsibilities: Administrative Support • Provide administrative support to the Care Management department, including the Director and Assistant Directors. • Enter units of services rendered into care management systems, quality-check the information provided by other department staff, and generate monthly and quarterly reports. • Assist in client assignments to members of the Care Management team. • Receive, track, and process departmental vendor invoices for financial reimbursement, and collaborate with the Finance Department to insure timely payments and quality control. • Compile and submit funder/internal reports, including all pertinent documentation for quality assurance purposes. Client Service Delivery • Provide case management, information, referrals, and advocacy to SAGE clients and participants. • Open cases, assess ongoing client needs, develop care plan strategies, conduct home visits, and accompany clients to scheduled appointments. • Conduct department intakes and registrations, complete phone screenings, and assist with mental health surveys. • Administer benefit and entitlement eligibility assessments, and assist clients with public benefits applications and other entitlement programs and opportunities. • Collect and enter all client service unit data and client notes in databases thoroughly and in a timely manner in accordance with organizational policies and regulatory mandates, as well as fulfilling all other reporting requirements. • Attend program events, organization meetings, funder trainings, and community outreach events. • Perform other tasks and duties as assigned. Preferred Qualifications: • B.A. in social work or a related field, or sufficient professional experience in a similar role • Demonstrated work experience in similar position involving equivalently complex data management, departmental compliance, and other pertinent administrative tasks • Knowledge of electronic records systems and Salesforce strongly preferred • Familiarity with LGBTQ+ communities • Spanish fluency strongly preferred • Strong written, proofreading and verbal communication skills • Computer-literate, with fluency in MS Office Suite, with particular emphasis on Excel (must be at least an intermediate user) • Self-starter with good organizational and time management skills • Ability to work independently and as a member of a team • Support for SAGE Mission and Core Values.
