Care Coordination Role We are hiring a Scheduling / Care Coordinator to join our growing nonmedical homecare company here is Carmel Indiana. The Schedule / Care Coordinator is responsible for creating & maintaining the weekly schedule for caregivers and clients. This position will work side by side office & supporting staff. They will coordinate & communicate with all staff & clients, create their schedules & availability, and ensure their files are accurate. They will receive on-the-job training with the office staff & the management team. This position is fast paced & requires the ability to communicate, prioritize, think critically, & problem solve. How You Will Make An Impact The Care Coordinator’s main function is to make sure all clients and caregivers are matched to the clients’ preferred schedule. The Care Coordinator MUST be able to build relationships and know the important characteristics of each caregiver to be able to match their skills and personalities with clients. Responsibilities of the Care Coordinator: • Building schedules • Finding caregivers for clients that need immediate attention • Manage caregiver call outs • Receiving and confirming availability of employees • Caregiver matching • Manage unconfirmed shifts • Scheduling adjustments • Confirming shifts • Managing client care for consistent caregivers • Documentation of various events, situations, changes, cancelations, and other occurrences • Answering phone calls • Documenting all client and caregiver information and interaction • Communicating employee concerns and issues with operations manager and/or RN • Communication with all staff members • Communicating with clients and client contacts What We Offer • Professional development assistance • Competitive benefits packages including medical, dental, vision, • Paid vacation • 401k for all employees 18 and over. What You Will Need: • · College degree : Associate or higher degree preferred. • · Excellent communication skills • · Enjoy problem solving • · Computer proficiency required • · Takes and active role in self-development through continuing education, utilizing resources within the health care system or elsewhere. • · Previous supervisory experience preferred or demonstration of progressive leadership responsibility is required. • · The ability to establish and maintain interpersonal and interdepartmental relationships. • · Must have one (1) year of experience with a frail or elderly population. • · The ability to guide and monitor home care assistants in the delivery of care.
