Full Description

SeaStar Senior Services provides in home care, companionship, and care management for seniors and vulnerable adults in Portland metro area, including Clark County, Washington. We are looking for a professional care manager who wants to be part of a company that is focused on providing quality, compassionate care to our clients. We're currently looking for someone with flexibility to start part time, about 10 hours a week, with the ability to take on more hours as new clients are brought on to service. This does require open availability to meet client needs during business hours and also take part in after hours on call rotation. If applicant is an Oregon Registered Nurse, there may be more hours available under our In Home Care department. Duties: • Perform comprehensive assessments, gathering information about health history, family and social dynamics, natural support systems, and environmental concerns and needs. • Create thorough care plans enlisting supports already in place and arranging access to other needed services. • Implement care plan actions to reach goals established during assessment and care planning. • Update assessments and care plans quarterly or as needed. • Respond in timely manner to changes in client's condition or environment • Determine housing needs and assist in placement for higher level of care or arranging resources for client safety to remain in their home • Attend care conferences and serve as liaison between facilities, family, and professional fiduciaries • Coordinate all medical appointments, specialty follow up visits, procedures, and obtain needed medications. • Obtain other needed professional services including legal, financial, care, and more. • Provide or arrange for transportation to appointments and social outings • Document all work performed on client's behalf with detailed notes and accurate time or provide monthly reports to responsible parties as desired • Participate in weekly on-call rotation for evenings and weekends. • Perform other related duties as assigned Qualifications: • Registered Nurse License OR Master's in Social Work degree REQUIRED • Experience working with seniors and adults with disabilities • Excellent driving record and own car • CPR certified or able to obtain • Good observational and writing skills • Must have excellent listening skills with critical thinking • Computer skills required Compensation: • 45 - 50.00 per hour DOE Job Types: Part-time, Per diem Expected hours: 8 to 10 per week to start, will increase as clients onboard We do drug tests and nationwide background checks
