Full Description

$55,000 a year. Compassionate and dedicated Caseworker with hands-on experience supporting children with intellectual and developmental disabilities in residential care settings. Committed to a childcentered, family-focused, and goal-directed approach that promotes dignity, empowerment, and self-determination. Skilled in developing individualized service plans, conducting risk assessments, and collaborating with interdisciplinary teams to ensure holistic care. Known for building strong partnerships with families, youth, and community members, and for maintaining a non-judgmental, culturally sensitive approach to care. Flexible, responsive, and deeply invested in improving the lives of vulnerable children and their families. Key Responsibilities Provide casework services to children with I/DD and their families, focusing on safety, permanency planning, and discharge readiness. Develop and implement individualized service plans in collaboration with multidisciplinary treatment teams. Conduct ongoing risk and safety assessments, making timely referrals to appropriate services and supports. Maintain thorough documentation including Family Assessment and Service Plans (FASPs), progress notes, psychosocial summaries, and court reports. Deliver crisis intervention and emotional support during behavioral and family emergencies. Act as liaison between families and agencies such as ACS, DSS, OCFS, OPWDD, and educational institutions. Conduct home assessments and field visits to ensure safe and supportive environments. Represented the agency in court hearings, preparing reports and following up on legal orders. Attend IEP meetings and collaborated with school personnel to support educational goals. Facilitated family and sibling visitation, maintaining consistent documentation and communication. Ensure proper intake and discharge procedures, supporting smooth transitions for youth. Qualifications Possess a desire to positively influence the lives of others. Strong advocacy, communication and organizational skills are necessary. Bachelors degree required in social work or a related field, master's in social work strongly preferred; other disciplines considered. Foster Care Experience Is Preferred. Ability to work flexible hours, including occasional weekend and evening hours Experience with the Intellectual or Developmentally Disabled population is preferred. Bi-lingual ability is a plus (Cantonese, Mandarin or Spanish) A Valid NYS Drivers License is required.
