Job Summary We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the highest quality of service across various settings, including assisted living, nursing homes, and home care. This position requires a strong background in case management and experience working with diverse populations, including children and seniors. Responsibilities • Coordinate and manage patient care plans, ensuring they meet the individual needs of each patient. • Conduct comprehensive assessments of patients’ medical histories and current health status. • Collaborate with multidisciplinary teams to develop effective treatment plans. • Supervise and support nursing staff in delivering high-quality patient care. • Provide education to patients and their families regarding health management and available resources. • Monitor patient progress and adjust care plans as necessary. • Maintain accurate documentation of patient interactions and care provided. • Ensure compliance with all regulatory requirements and organisational policies. • Advocate for patients’ needs within the healthcare system. Qualifications • Registered Nurse (RN) qualification with a valid nursing licence. • Proven experience in case management, particularly in senior care, hospice care, or home care settings. • Strong background in assisted living, nursing home experience, or ICU experience is highly desirable. • Knowledge of anatomy and medication administration is essential. • Experience working with individuals with disabilities, dementia care, Alzheimer's care, or toddler care is advantageous. • Excellent team management skills with prior supervising experience preferred. • Strong communication skills and the ability to work collaboratively with healthcare professionals and families. • Familiarity with laboratory procedures and medical imaging is a plus. • A compassionate approach to patient care with a commitment to improving patients' quality of life.
