Bachelor's degree required (Master's preferred), at least 1 year of case management experience, knowledge of addiction treatment preferred, ability to drive and do outreach, Spanish preferred but not required.
HOUSING SUPPORT CASE MANAGER DESCRIPTION The HSS case manager position at Riverbend is a DPH/BSAS funded position that will provide case management and support to individuals living in the 555 Merrimack St. Recovery Supported Housing who are struggling with substance use. The HSS case manager reports directly to the Director of Operations. Job Responsibilities: • Offer case management to the residents at 555 Merrimack St. Housing project • Utilize Riverbend’s electronic health records (EHR) to document all intake, case management and treatment coordination with providers. • Conduct psycho-social intakes with identified clients to identify treatment and case management needs. • Complete client-focused treatment plans and establish reasonable goals for clients to work on. • Carry a caseload of up to 18 clients and meet with clients regularly either in their home or in the office to provide support and assistance in areas of need. • Responsible for recruiting of clients at the 555 Merrimack St location • Provide case management services which may include connecting individual with community resources, making supported referrals, assisting individuals to complete a variety of tasks of daily living, teaching usable life and coping skills, and providing education about addiction and recovery. • Responsible for filling in at Riverbend’s residential treatment programs as needed and as directed by supervisor. • Attending meetings and trainings in the community as needed and all monthly mandatory Relias and other Riverbend assigned trainings. • Adhere to 555 Merrimack St. location office hours. • Attend and assisting with the facilitation of community meetings at 555 Merrimack St. • Collaboration with the other service providers at the 555 Merrimack St. site. • Facilitating psycho-education groups focused on addiction related topics as needed.
