Role Overview: As a Medication Management Specialist, you will play a vital role in ensuring the safe and effective administration of medications for clients with IDD. This position requires a Licensed Practical Nurse (LPN) who will monitor medical needs, coordinate care, and support clients and their families in managing medication regimens. What You'll Do: - You will monitor and manage medication administration for clients with IDD to ensure compliance and safety. - You will collaborate with healthcare providers, families, and care teams to develop and update medication plans. - You will conduct regular assessments and documentation of clients’ medication responses and health status. - You will provide training and support to caregivers and staff on medication protocols and best practices. - You will ensure adherence to regulatory standards and organizational policies related to medication management. - You will participate in quality improvement initiatives to enhance medication safety and client outcomes. - You will respond promptly to medication-related concerns and emergencies. - You will maintain accurate records and reports in compliance with healthcare regulations. What You Bring: - You must hold a current Licensed Practical Nurse (LPN) license. - You should have at least 2 years of experience in medication management or nursing, preferably with IDD populations. - You need strong knowledge of medication administration, safety protocols, and healthcare regulations. - You must possess excellent communication and interpersonal skills to work effectively with clients, families, and healthcare teams. - You should be proficient in documentation and electronic health record (EHR) systems. Bonus Points If You Have: - Experience working in community-based healthcare or disability services. - Familiarity with intellectual and developmental disabilities (IDD) care. - Certification in medication administration or related specialties. - Ability to work independently in a remote work environment.
