Job Title: Specialist, Client Services - Medication Management Specialist Company Overview: Redwood Family Care Network is a leading healthcare organization dedicated to providing compassionate and comprehensive care to individuals with intellectual and developmental disabilities (IDD). With a strong commitment to improving quality of life, Redwood Family Care Network supports families and individuals through innovative programs and personalized services. Role Overview: As a Medication Management Specialist, you will play a vital role in ensuring the safe and effective administration of medications for individuals with IDD. This position requires a Licensed Practical Nurse (LPN) who is detail-oriented and passionate about client care. You will work closely with healthcare teams to monitor medication plans and support client health outcomes. What You'll Do: - You will monitor and manage medication administration for clients with IDD to ensure compliance with prescribed treatments. - You will collaborate with healthcare providers and families to update and maintain accurate medication records. - You will conduct regular assessments to identify potential medication-related issues and intervene as necessary. - You will provide training and support to caregivers on proper medication handling and administration. - You will document all medication-related activities in compliance with regulatory standards. - You will participate in team meetings to discuss client progress and medication management strategies. - You will ensure adherence to safety protocols and organizational policies regarding medication management. - You will support continuous quality improvement initiatives within the Medication Management Program. What You Bring: - You bring a current and valid LPN license with at least 2 years of experience in medication management or a related healthcare field. - You bring strong knowledge of medication administration procedures and safety standards. - You bring excellent communication skills to effectively interact with clients, families, and healthcare teams. - You bring the ability to work independently in a remote work environment while managing multiple responsibilities. - You bring proficiency in electronic health records (EHR) and documentation practices. Bonus Points If You Have: - Experience working with individuals with intellectual and developmental disabilities (IDD). - Certification in medication administration or related specialized training. - Familiarity with state and federal regulations related to medication management. - Experience in a community-based healthcare setting.
