Role Overview: As a Medication Management Specialist, you will play a vital role in ensuring the safe and effective administration of medications for individuals with IDD. This full-time, work-from-home position requires a licensed practical nurse (LPN) who is passionate about client care and medication safety. What You'll Do: - You will monitor and manage medication administration for clients with IDD to ensure compliance with medical protocols. - You will collaborate with healthcare providers, families, and care teams to coordinate medication plans. - You will conduct regular assessments and documentation of medication effects and client health status. - You will provide training and support to caregivers on proper medication handling and administration. - You will ensure adherence to regulatory standards and organizational policies related to medication management. - You will participate in quality improvement initiatives to enhance client safety and care outcomes. - You will respond promptly to medication-related concerns and emergencies. - You will maintain accurate records and reports in compliance with healthcare regulations. What You Bring: - Licensed Practical Nurse (LPN) certification with current licensure. - Minimum of 2 years of experience in medication management or nursing, preferably with IDD populations. - Strong knowledge of medication administration protocols and safety standards. - Excellent communication and interpersonal skills to work effectively with clients, families, and healthcare teams. - Ability to work independently in a remote work environment while managing multiple priorities. Bonus Points If You Have: - Experience working with individuals with intellectual and developmental disabilities. - Familiarity with electronic health records (EHR) systems. - Additional certifications related to medication management or nursing. - Previous experience in a community healthcare or family care network setting.
