Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care and resources for clients receiving specialized waiver programs. Your efforts will ensure that clients receive the appropriate services to improve their health outcomes and quality of life. What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver program requirements. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as necessary to meet evolving needs. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to necessary services and supports. - You will educate clients and families about available resources and waiver program benefits. - You will collaborate with interdisciplinary teams to promote holistic care approaches. - You will participate in quality improvement initiatives to enhance service delivery. What You Bring: - Minimum of 2 years experience in case management or related healthcare coordination roles. - Knowledge of waiver programs and eligibility criteria. - Strong organizational and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency with electronic health records and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver services. - Background in behavioral health or developmental disabilities. - Bilingual skills, particularly in Spanish. - Training in motivational interviewing or similar client engagement techniques.
