Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care and resources for clients receiving specialized health services. Your efforts will ensure that clients receive timely, effective, and holistic support tailored to their unique needs. What You'll Do: - You will assess client needs and develop individualized care plans to support their health and wellness goals. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as necessary to ensure optimal outcomes. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to necessary services and supports. - You will participate in interdisciplinary team meetings to collaborate on client care strategies. - You will provide education and support to clients and their families regarding available services and resources. - You will ensure adherence to all organizational policies and procedures related to case management. What You Bring: - Minimum of 2 years of experience in case management or a related healthcare field. - Knowledge of waivered services and community-based healthcare programs. - Strong organizational and communication skills to manage multiple cases effectively. - Ability to work independently and as part of a multidisciplinary team. - Proficiency in electronic health records and case management software. - Relevant certification or licensure in social work, nursing, or related discipline preferred. Bonus Points If You Have: - Experience working with populations requiring waivered services. - Familiarity with Medicaid and other funding sources for healthcare services. - Bilingual abilities to support diverse client populations. - Training in trauma-informed care or behavioral health.
