Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care for clients receiving specialized waiver programs. Your focus will be on ensuring clients access necessary services, advocating for their needs, and facilitating communication among healthcare providers, clients, and families. What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver service requirements. - You will coordinate and monitor service delivery to ensure compliance with program guidelines. - You will advocate for clients to secure appropriate resources and support. - You will maintain accurate and timely documentation of client interactions and service plans. - You will collaborate with multidisciplinary teams to optimize client outcomes. - You will provide education and support to clients and their families regarding available services. - You will participate in quality improvement initiatives to enhance service effectiveness. - You will stay informed about changes in waiver programs and relevant healthcare regulations. What You Bring: - Minimum of 2 years experience in case management or related healthcare coordination roles. - Knowledge of waiver programs and healthcare service delivery systems. - Strong organizational and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency in documentation and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver services. - Background in behavioral health or developmental disabilities. - Bilingual abilities to support diverse client populations. - Training in trauma-informed care or motivational interviewing.
