What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver service requirements. - You will coordinate and monitor service delivery to ensure compliance with program guidelines. - You will advocate for clients to secure appropriate resources and support. - You will maintain accurate and timely documentation of client interactions and service plans. - You will collaborate with multidisciplinary teams to optimize client outcomes. - You will provide education and support to clients and their families regarding available services. - You will participate in quality improvement initiatives to enhance service effectiveness. - You will stay informed about changes in waiver programs and relevant healthcare regulations. What You Bring: - Minimum of 2 years experience in case management or related healthcare coordination roles. - Knowledge of waiver programs and healthcare service delivery systems. - Strong organizational and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency in documentation and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver services. - Background in behavioral health or developmental disabilities. - Bilingual abilities to support diverse client populations. - Training in trauma-informed care or motivational interviewing.
