What You Bring: - You have a minimum of 2 years of experience in case management or a related healthcare field. - You possess strong knowledge of waivered services and community-based healthcare. - You have excellent communication and interpersonal skills. - You are proficient in electronic health records and case management software. - You hold a relevant certification or licensure in social work, nursing, or related discipline. Bonus Points If You Have: - Experience working with behavioral health or substance use disorder populations. - Familiarity with Medicaid waivers and funding mechanisms. - Bilingual abilities, especially in Spanish. - Experience in a fast-paced healthcare environment.
