Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care and support for clients receiving specialized health services. Your efforts will ensure that clients access the resources they need while maintaining compliance with regulatory standards. What You'll Do: - You will assess client needs and develop individualized care plans. - You will coordinate with healthcare providers, social services, and community resources. - You will monitor client progress and adjust care plans as necessary. - You will maintain accurate and timely documentation of all case management activities. - You will advocate for clients to ensure they receive appropriate services. - You will participate in interdisciplinary team meetings to discuss client care. - You will ensure compliance with all relevant policies, procedures, and regulations. - You will provide education and support to clients and their families. What You Bring: - You have a minimum of 2 years of experience in case management or a related healthcare field. - You possess strong knowledge of waivered services and community-based healthcare. - You have excellent communication and interpersonal skills. - You are proficient in electronic health records and case management software. - You hold a relevant certification or licensure in social work, nursing, or related discipline. Bonus Points If You Have: - Experience working with behavioral health or substance use disorder populations. - Familiarity with Medicaid waivers and funding mechanisms. - Bilingual abilities, especially in Spanish. - Experience in a fast-paced healthcare environment.
