Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care and resources for clients receiving specialized waiver programs. Your efforts will ensure that clients receive the appropriate services to improve their health outcomes and quality of life. What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver service requirements. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as necessary to meet evolving needs. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to necessary services and supports. - You will collaborate with multidisciplinary teams to optimize client outcomes. - You will participate in training and professional development to stay current with waiver program policies. - You will assist clients and families in navigating healthcare systems and resources. What You Bring: - Minimum of 2 years experience in case management or related healthcare/social services roles. - Knowledge of waiver programs and eligibility criteria. - Strong organizational and communication skills. - Ability to work independently and as part of a team. - Proficiency with electronic health records and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver services. - Bilingual abilities to support diverse client populations. - Background in mental health or developmental disabilities services. - Experience in community outreach and resource coordination.
