What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver program requirements. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as needed to meet evolving needs. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to appropriate services and supports. - You will educate clients and families about waiver services and available resources. - You will collaborate with interdisciplinary teams to promote holistic care approaches. - You will participate in quality improvement initiatives to enhance service delivery. What You Bring: - Minimum of 2 years experience in case management or related healthcare/social services field. - Knowledge of waiver programs and eligibility criteria. - Strong organizational and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency with electronic health records and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver programs. - Bilingual abilities to support diverse client populations. - Background in mental health or developmental disabilities services. - Training in motivational interviewing or client engagement techniques.
