What You Bring: - You have a minimum of 2 years of experience in case management or a related healthcare role. - You possess strong knowledge of waivered services and community-based care. - You have excellent communication and interpersonal skills. - You are proficient in electronic health records and case management software. - You hold a relevant certification or licensure in social work, nursing, or a related field. Bonus Points If You Have: - Experience working with Medicaid waivered programs. - Bilingual abilities, especially in Spanish. - Background in mental health or substance use disorder services. - Strong problem-solving and crisis intervention skills.
