Role Overview: As a Case Manager for Waivered Services, you will play a crucial role in coordinating care for clients receiving specialized health services. Your focus will be on ensuring clients access the appropriate resources and support to improve their health outcomes and quality of life. What You'll Do: - You will assess client needs and develop individualized care plans. - You will coordinate with healthcare providers, social services, and community resources. - You will monitor client progress and adjust care plans as necessary. - You will advocate for clients to ensure they receive timely and effective services. - You will maintain accurate and up-to-date documentation of client interactions and care plans. - You will facilitate communication between clients, families, and care teams. - You will participate in team meetings and contribute to continuous improvement initiatives. - You will ensure compliance with all regulatory and organizational policies. What You Bring: - You have a minimum of 2 years of experience in case management or a related healthcare role. - You possess strong knowledge of waivered services and community-based care. - You have excellent communication and interpersonal skills. - You are proficient in electronic health records and case management software. - You hold a relevant certification or licensure in social work, nursing, or a related field. Bonus Points If You Have: - Experience working with Medicaid waivered programs. - Bilingual abilities, especially in Spanish. - Background in mental health or substance use disorder services. - Strong problem-solving and crisis intervention skills.
