Role Overview: As a Case Manager for Waivered Services, you will play a critical role in coordinating care and resources for clients receiving specialized waiver programs. What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver program requirements. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as necessary to meet evolving needs. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to necessary services and supports. - You will collaborate with interdisciplinary teams to optimize client outcomes. - You will participate in training and professional development to stay current with waiver program policies. - You will assist clients and families in navigating healthcare systems and resources. What You Bring: - Minimum of 2 years experience in case management or related healthcare coordination roles. - Knowledge of waiver services and Medicaid programs. - Strong organizational and communication skills. - Ability to work independently and as part of a multidisciplinary team. - Proficiency with electronic health records and case management software. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with waiver populations or in community-based healthcare. - Bilingual abilities to support diverse client populations. - Familiarity with state and federal healthcare regulations. - Background in mental health or behavioral health services.
