Role Overview: As a Case Manager for Waivered Services, you will play a critical role in managing and coordinating care for clients receiving specialized waiver programs. Your efforts will ensure that clients receive the appropriate services and support to enhance their quality of life and achieve their health goals. What You'll Do: - You will assess client needs and develop individualized care plans tailored to waiver program requirements. - You will coordinate with healthcare providers, social services, and community resources to facilitate comprehensive care. - You will monitor client progress and adjust care plans as necessary to meet evolving needs. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will advocate for clients to ensure access to necessary services and supports. - You will collaborate with multidisciplinary teams to promote holistic care approaches. - You will participate in training and professional development to stay current with waiver program policies. - You will support clients and families through education and resource linkage. What You Bring: - Minimum of 2 years experience in case management or related healthcare coordination roles. - Knowledge of waiver services and community-based care models. - Strong organizational and communication skills. - Ability to work independently and as part of a team. - Proficiency with electronic health records and documentation systems. - Relevant certification or licensure in social work, nursing, or related field preferred. Bonus Points If You Have: - Experience working with Medicaid waiver programs. - Background in behavioral health or developmental disabilities services. - Bilingual abilities to support diverse client populations. - Experience with crisis intervention and conflict resolution.
