Role Overview: As a Case Manager at Radias Health, you will play a critical role in coordinating patient care, ensuring access to necessary resources, and supporting patients through their healthcare journeys. What You'll Do: - You will assess patient needs and develop individualized care plans. - You will coordinate with healthcare providers, social services, and community resources. - You will monitor patient progress and adjust care plans as necessary. - You will advocate for patients to ensure they receive appropriate services. - You will document all case management activities accurately and timely. - You will educate patients and families about health conditions and available resources. - You will collaborate with interdisciplinary teams to support holistic care. - You will participate in quality improvement initiatives to enhance service delivery. What You Bring: - Minimum of 2 years experience in case management or related healthcare role. - Strong knowledge of healthcare systems, social services, and community resources. - Excellent communication and interpersonal skills. - Ability to manage multiple cases and prioritize effectively. - Proficiency with electronic health records and case management software. - Relevant certification such as CCM (Certified Case Manager) is preferred. Bonus Points If You Have: - Experience working with diverse populations and underserved communities. - Background in behavioral health or chronic disease management. - Bilingual abilities, especially in Spanish or other commonly spoken languages. - Experience with data analysis and reporting for case management outcomes.
