What You Bring: - Minimum of 2 years experience in case management or related healthcare role. - Strong knowledge of healthcare systems, social services, and community resources. - Excellent communication and interpersonal skills. - Ability to manage multiple cases and prioritize effectively. - Proficiency with electronic health records and case management software. - Relevant certification such as CCM (Certified Case Manager) is preferred. Bonus Points If You Have: - Experience working with diverse populations and underserved communities. - Background in behavioral health or chronic disease management. - Bilingual abilities, especially in Spanish or other commonly spoken languages. - Experience with data analysis and reporting for case management outcomes.
