Full Description

This is a combined role encompassing Case Management and Staff Education. Uses Case Management Guiding Principles in partnering with patients and physicians to assure access, services, and transition for the continuum of care. Works as team member with health care professionals, community resources, liaisons, and families to provide coordinated admission and discharge planning. Advocates and supports the case management process using the Physician-Centric Care Management Model as a guideline. Providence caregivers are not simply valued – they’re invaluable. Upon hire: Alaska Registered Nurse License. 2 years Clinical nursing experience. Preferred qualifications: 3 years Clinical nursing experience. 1 year Utilization Management-review and Discharge-planning experience.
