Job Summary Join our dedicated healthcare team as a Certified Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and independence. Duties • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts. • Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. • Administer medications and perform catheter care following medical instructions and HIPAA privacy regulations. • Support clients with specialized needs such as Alzheimer’s care, dementia care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. • Prepare nutritious meals, assist with cooking and cleaning tasks to maintain a safe living environment, including laundry and light housekeeping. • Maintain detailed medical documentation and ensure compliance with healthcare standards while respecting patient confidentiality. • Use medical terminology confidently when recording patient information or communicating with healthcare professionals. Requirements • Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA). • Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments. • Strong skills in patient observation, vital signs measurement, medication administration, and infection control practices. • Knowledge of medical procedures such as CPR certification, catheter care, hospice support techniques, and dementia or Alzheimer’s care. • Experience working with people with disabilities or developmental challenges preferred. • Ability to operate EHR/EMR systems efficiently for accurate documentation. • Excellent communication skills to work effectively with patients, families, and healthcare teams while maintaining HIPAA compliance. • Physical stamina to assist with mobility support and perform caregiving duties throughout the shift.
