Patient Care Coordinator Lehi, UT Hybrid and Remote Who We Are: Proactive Health is a dynamic and innovative company dedicated to improving patient care for seniors through cutting-edge technology and compassionate service. By blending advanced biometric monitoring with the strength of personal connections, our goal is to empower seniors and their healthcare providers with the data, knowledge, and tools to take charge of their health, reducing readmissions to the hospital and enhancing their quality of life. We are seeking a compassionate and detail-oriented Patient Care Coordinator to join our healthcare team. In this role, you will be responsible for managing the day to day care coordination of patients who are using devices to track their biometrics at home. Your primary responsibility will be to ensure that patients receive timely and personalized attention based on their monitored health data, helping to provide optimal care and support. It is important that the Patient Care Coordinator always provides excellent customer service and promotes the benefits of being proactive in health through dress and behaviors. Additionally, the Patient Care Coordinator's role is to build fantastic rapport with the patient to help them feel heard and cared for, and to report all pertinent information to the correct medical professional as outlined by the medical director. What You'll Do: - Daily Monitoring: Regularly check and interpret data from at-home monitoring devices (e.g., blood pressure, heart rate, oxygen levels) to track patients' health progress and identify any concerns. - Daily Communication: Conduct daily phone calls with patients to discuss their vital signs, address any concerns, and provide additional resources if applicable to the patient and their needs. - Build Rapport with Patients: Foster positive, professional relationships with patients by maintaining a friendly and empathetic demeanor in every interaction. Build rapport through active listening and remembering personal details to make each patient feel valued and cared for. - Personalized Care: Take note of key personal information (e.g., preferred communication styles, lifestyle, health goals) and use this to provide individualized care, making sure each patient feels uniquely cared for. Develop an efficient system for recording and recalling these details as needed. - Address Patient Concerns: Answer all questions and concerns patiently and professionally. Ensure that patients feel heard and respected during each interaction, offering guidance or escalating issues to the relevant healthcare professional when needed. - Managing Own Schedule: Maintain an organized call / visit schedule to ensure timely check-ins with each patient - Collaboration: Work closely with other healthcare professionals to provide care. Communicate any significant changes in a patient's health or vital signs to the appropriate medical team. - Documentation: Accurately document each patient interaction, recording any concerns, vital sign updates, and personal notes for future reference. Ensure patient records are kept confidential and up to date What You Need: - Must have prior experience in healthcare-CNA or Medical Assistant, patient care coordination, or a strong customer service background. - Strong communication skills, both verbal and written. - Excellent interpersonal skills with the ability to build rapport and trust with patients. - Empathetic, patient, and able to maintain a positive attitude even under pressure. - Strong ability to learn new technology and platforms. - Organized and detail-oriented with strong time management skills. - Ability to work independently and achieve consistent performance metrics. - Knowledge of HIPAA regulations and the importance of patient confidentiality. - Spanish Speaking is a plus.
