Key Responsibilities • Patient Care Coordination: Manage and coordinate patient care across various Preferred Provider settings to ensure seamless and efficient care delivery. Serve as the primary liaison between the Preferred Provider network and Pinnacle Home Care. • Compliance: Ensure all activities comply with Preferred Provider guidelines and regulations. • Care Plan Development: Collaborate with Preferred Provider professionals to develop and implement individualized care plans for patients. • Patient Education: Educate patients and their families about their conditions, treatment options, and care plans. • Data Management: Maintain accurate and up-to-date patient records and documentation. • Quality Improvement: Identify opportunities for quality improvement and implement strategies to enhance patient outcomes. • Communication: Serve as the primary point of contact for patients, families, and healthcare providers within the Preferred Provider network. • Resource Coordination: Connect patients with community resources and support services as needed. • Reporting: Prepare and present reports on patient outcomes, care coordination activities, and compliance with Preferred Provider guidelines. Qualifications • Education: High School Diploma. • Experience: Care coordination, case management, or a related field. Experience working within a Preferred Provider network is highly desirable. • Skills: Strong organizational, communication, and interpersonal skills. Ability to work collaboratively with a multidisciplinary team. • Certifications: Relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ), are a plus.
