We're seeking a detail-oriented healthcare professional who can manage short-term case coordination with focus on care continuity and member advocacy. As our Case Management Coordinator, you'll telephonically manage a caseload of 20-25 cases while utilizing multiple data sources to implement comprehensive care plans that ensure seamless healthcare experiences. What makes this role different ✓ Member advocacy focus: Serve as dedicated coordinator ensuring members receive appropriate, timely care through complex healthcare systems ✓ Data integration expertise: Utilize multiple information sources including claims data, medical records, and provider conversations to create comprehensive care plans ✓ Cross-functional collaboration: Build working relationships with internal departments, healthcare providers, clients, and patients for optimal outcomes ✓ Short-term intervention impact: Focus on care coordination and continuity-of-care that makes immediate, meaningful difference in member experiences What You'll Actually Do Coordinate comprehensive care: Manage telephonic caseload of 20-25 cases focused on care coordination and continuity-of-care interventions that ensure seamless member experiences. Synthesize complex information: Utilize multiple sources including paid claims data, customer service notes, clinical guidelines, medical records, member conversations, and provider discussions to implement supervisor-outlined care plans. Build strategic relationships: Establish and maintain working relationships with internal departments, healthcare providers, clients/groups, and patients to facilitate optimal care coordination. Maintain detailed documentation: Complete comprehensive documentation of care plan progress in Eldorado and UM Web while submitting finalized reports every 4 weeks reflecting completed work. Meet performance standards: Achieve productivity, quality, and turnaround time requirements on daily, weekly, and monthly basis while maintaining focus on member outcomes. Qualifications What You Bring to Our Mission The foundational experience: High school diploma or GED required 2-3 years prior experience in customer service, claims, and/or triage Coordination of care experience in insurance or hospital setting The technical expertise: Computer proficiency and working knowledge of Microsoft Office Suite and Microsoft Outlook required Knowledge of medical claims and ICD-10, CPT, HCPCS coding preferred Background in medical terminology The professional competencies: Ability to critically evaluate data and provide accurate patient assessment with capability to establish treatment plans Excellent interpersonal and communication skills with strong customer orientation Good time management skills with highly organized approach Strong analytical abilities for synthesizing complex healthcare information into actionable care coordination strategies Why You'll Love It Here We believe in creating meaningful work experiences that fit your life—whether you're building your career, balancing other commitments, or seeking purposeful part-time opportunities.
