Summary We are seeking a dedicated Assistant Program Manager to join our team in Washington County. In this role, you will play a crucial part in enhancing the lives of individuals with developmental disabilities by overseeing program development and implementation. Your leadership will be vital in fostering independence and personal growth among our clients. Duties and Responsibilities: 1. Individual Support • Assist in ensuring all services align with the supported individuals' service agreements and Individual Support Plans (ISPs). • Provide direct services to supported individuals to help them achieve their goals. • Work with supported individuals one-on-one and in group settings, tracking progress, challenges, and achievements. • Discuss goals and services with supported individuals monthly to align with their evolving needs. • Maintain accurate and timely progress notes per Oregon Administrative Rules (OAR). • Support program and documentation development in collaboration with the Program Manager and management team. • Ensure adherence to safety plans, protocols, and regulatory compliance. 2. Staff Supervision & Training • Support DSP’s ensuring they understand and follow ISPs. • Conduct ride-along with Direct Support Professionals (DSPs) and document evaluations at least twice monthly. • Support the Program Manager in monthly staff meetings. • Participate in weekly Structure Meetings, Staff Meetings, and other program-related meetings. • Assist in reviewing progress notes, uploading documents, and ensuring staff training is completed in line with state-regulated timelines and requirements. • Update insurance records and provide documentation support for DSP roles and other tasks within Aaniie. 3. Scheduling & Coordination • Collaborate with the Program Manager, staff, and Social Enrichment Coordinator to schedule services in Aaniie. • Assist in the scheduling and coordination of supported individuals and providers. 4. Administrative & Operational Support • Maintain accurate records of hours worked, mileage, and daily activities. • Assist in performance reviews, provide check-ins, and implement corrective actions as needed. • Develop necessary ISP documents, i.e. 1-page profiles, desired outcomes, action plans, interaction guidelines, safety plans, and protocols to ensure compliance. • Support the Program Manager with ongoing projects and administrative tasks. 5. Community Engagement & Support • Provide transportation or assist supported individuals with public transportation to activities and community outings within Oregon. • Act as a positive role model and advocate for supported individuals. • Collaborate with families, caregivers, and external agencies to enhance social integration and independence. 6. Communication and Collaboration: • Following Chain of Command within PDXconnects. • Serve as a positive role model and communicate regularly with individuals and/or their family members regarding schedules and reminders. • Collaborate with other staff members to coordinate goals and manage contract hours for supported individuals. 7. Other Duties • Perform other duties as assigned to ensure quality service delivery and program success. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Ability to perform these operations using units of American money and weight measurement, volume, and distance. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills: To perform this job successfully, an individual should have knowledge of word processing software, internet software, smart phone, apps and use of electronic signatures, scanning and saving documents such as PDFs, using all Microsoft products, One Drive, and Microsoft Calendar, SharePoint, etc. use internet to search activities and other resources. Education/Experience: High school diploma or general education degree (GED); or one to two year’s related experience and/or training; or equivalent combination of education and experience. Certificates and Licenses required by OAR 411-323 and Standard Agency Model: · Valid driver’s license. · Valid insurance in compliance with the laws of the Department of Motor Vehicles. · Certified in CPR and First Aid by a recognized training agency. · Mandatory abuse training and notification of mandatory reporter status annually. · 24 hours of job-related in-service training annually. · OIS Oregon Intervention Systems every two years or as needed. · Any other training that is assigned, required per OAR’s, or provided to employees from PDXconnects for the overall growth and services provided by PDXconnects.
