Role Overview: As a Self-Directed Service Coordinator, you will play a vital role in partnering with individuals and their support networks to design, implement, and monitor personalized service plans. Your work will ensure that each person’s unique needs and goals are met with compassion and professionalism. What You'll Do: - You will collaborate closely with individuals and their families to develop tailored service plans that promote independence and quality of life. - You will coordinate and monitor services to ensure compliance with state regulations and organizational standards. - You will advocate for individuals’ rights and preferences, ensuring their voices are central in decision-making. - You will maintain accurate and timely documentation of service plans, progress notes, and reports. - You will facilitate communication between service providers, individuals, and families to resolve issues and enhance service delivery. - You will participate in ongoing training and professional development to stay current with best practices. - You will assist individuals in accessing community resources and supports to foster inclusion. What You Bring: - Minimum of 3 years experience in service coordination, case management, or a related field supporting individuals with developmental disabilities. - Strong knowledge of self-directed service models and relevant state regulations. - Excellent interpersonal and communication skills to build trusting relationships. - Ability to manage multiple cases effectively with strong organizational skills. - Proficiency in documentation and reporting using electronic record systems. Bonus Points If You Have: - Certification in developmental disabilities or related fields. - Experience working within New Hampshire’s developmental services system. - Bilingual abilities to support diverse communities. - Background in advocacy or community outreach.
