Role Overview: As a Self-Directed Service Coordinator, you will play a vital role in partnering with individuals and their support networks to design, implement, and manage personalized service plans. Your work will ensure that each person’s unique needs and goals are met with compassion and professionalism. What You'll Do: - You will collaborate closely with individuals and their families to develop tailored service plans that promote independence and quality of life. - You will coordinate and monitor services, ensuring compliance with state regulations and organizational standards. - You will advocate for individuals’ rights and preferences, fostering self-determination and empowerment. - You will maintain accurate and timely documentation of service plans, progress notes, and other required records. - You will facilitate communication among service providers, families, and community resources. - You will participate in ongoing training and professional development to stay current with best practices. - You will assist individuals in navigating available resources and funding options. What You Bring: - Minimum of 2 years experience in case management, social work, or a related field supporting individuals with developmental disabilities. - Strong knowledge of self-directed service models and person-centered planning. - Excellent communication and interpersonal skills to build trusting relationships. - Ability to manage multiple cases effectively and maintain detailed records. - Proficiency with Microsoft Office and case management software. Bonus Points If You Have: - Certification in Developmental Disabilities or related credentials. - Experience working within New Hampshire’s developmental services system. - Bilingual abilities, particularly in Spanish or French. - Background in advocacy or community organizing.
