At least 2 years of experience in service coordination or related fields, strong knowledge of self-directed service models, excellent communication and organizational skills, and proficiency in documentation systems.

Role Overview: As a Self-Directed Service Coordinator, you will play a vital role in partnering with individuals and their support networks to design, implement, and monitor personalized service plans. Your work will ensure that each person’s unique needs and goals are met with compassion and professionalism. What You'll Do: - You will collaborate closely with individuals and their families to develop tailored service plans that promote independence and quality of life. - You will coordinate and monitor services, ensuring compliance with state regulations and organizational standards. - You will advocate for individuals’ rights and preferences, facilitating access to community resources and supports. - You will maintain accurate and timely documentation of service plans, progress notes, and other required records. - You will conduct regular visits and assessments to evaluate the effectiveness of services and make necessary adjustments. - You will work as part of a multidisciplinary team to support holistic care and continuous improvement. - You will provide education and guidance to individuals and families about self-directed service options and processes. What You Bring: - Minimum of 2 years experience in service coordination, case management, or a related field supporting individuals with developmental disabilities. - Strong knowledge of self-directed service models and relevant state regulations. - Excellent communication and interpersonal skills to build trusting relationships. - Ability to manage multiple cases effectively with strong organizational skills. - Proficiency in documentation and data management systems. Bonus Points If You Have: - Experience working in community-based disability services. - Certification in developmental disabilities or related fields. - Familiarity with Medicaid waiver programs and funding mechanisms. - Bilingual abilities or experience working with diverse populations.
