Job Title: Self-Directed Service Coordinator Company Overview: PathWays of the River Valley has been a trusted provider of support services for individuals with developmental disabilities and their families in Sullivan and Lower Grafton counties of New Hampshire for over 50 years. Role Overview: As a Self-Directed Service Coordinator, you will play a vital role in partnering with individuals and their support networks to design, implement, and manage personalized service plans. Your work will ensure that each person’s unique needs and goals are met with dignity and respect, promoting autonomy and community inclusion. What You'll Do: - You will collaborate closely with individuals and their families to develop tailored service plans that reflect their preferences and aspirations. - You will coordinate resources and services, ensuring timely and effective delivery. - You will monitor and evaluate service outcomes, making adjustments as needed to optimize support. - You will advocate for individuals’ rights and access to community resources. - You will maintain accurate and up-to-date documentation in compliance with regulatory standards. - You will facilitate communication among service providers, individuals, and families to ensure cohesive support. - You will participate in ongoing training and professional development to stay current with best practices. What You Bring: - Minimum of 3 years experience in service coordination or case management, preferably in developmental disabilities or related fields. - Strong knowledge of community resources, service delivery models, and regulatory requirements. - Excellent interpersonal and communication skills to build trusting relationships. - Ability to manage multiple cases effectively with strong organizational skills. - Proficiency in documentation and data management systems. Bonus Points If You Have: - Experience with self-directed service models and person-centered planning. - Familiarity with New Hampshire state regulations related to developmental disabilities. - Certification in social work, counseling, or related disciplines. - Bilingual abilities or experience working with diverse populations.
