Job Title: Self-Directed Service Coordinator Company Overview: PathWays of the River Valley has been a trusted provider of support services for individuals with developmental disabilities and their families in Sullivan and Lower Grafton counties of New Hampshire for over 50 years. We are committed to empowering individuals to live fulfilling lives through personalized care and community integration. Role Overview: As a Self-Directed Service Coordinator, you will partner closely with individuals and their support networks to design, implement, and monitor personalized service plans. Your role is essential in promoting independence and enhancing quality of life for those we serve. What You'll Do: - You will collaborate with individuals and their families to develop tailored service plans that meet their unique needs and goals. - You will coordinate and monitor services to ensure effective implementation and compliance with regulatory standards. - You will advocate for individuals to access community resources and supports. - You will maintain accurate and timely documentation of service activities and progress. - You will facilitate communication among service providers, individuals, and families. - You will participate in ongoing training and professional development to stay current with best practices. - You will assist in resolving challenges and barriers to service delivery. What You Bring: - Minimum of 2 years experience in service coordination or a related field supporting individuals with developmental disabilities. - Strong knowledge of community resources and service systems in New Hampshire. - Excellent interpersonal and communication skills to build trusting relationships. - Ability to manage multiple cases and prioritize tasks effectively. - Proficiency in documentation and case management software. Bonus Points If You Have: - Certification in developmental disabilities or related areas. - Experience working in self-directed service models. - Bilingual skills, particularly in Spanish. - Background in social work or counseling.
