Home Health Occupational Therapist – Full time – You’ll provide one-on-one care to geriatric patients in their homes in and around Downtown Chicago. Employment Type - Full time basis (W2 and 1099 options available) Schedule - OTs can choose coverage areas + availability for days and visits/week. Competitive Compensation • Estimated Average Rate: $64/hr. –$68/hr. (Per visit Model) • Pay varies up to $120/SOC based on employment type (W2 or 1099) and the type of visit performed like Initial Evaluation, Follow-Up Visits, Start of Care (SOC) and pay for all Other OASIS Visits How this works: You are paid based on the type of visit you complete — each visit has its own rate. When you average a normal caseload together, most OTs earn $64/hr.–$68/hr. as based on per visit Model, and earnings may be higher if you complete a larger number of SOCs or evaluations. Key Responsibilities: • Deliver in-home occupational therapy services • Evaluate patient mobility, strength, and function to develop treatment goals • Implement effective rehabilitation and therapeutic exercise programs for adult and geriatric patients • Educate patients and families on recovery plans and preventive care • Maintain accurate and timely home health care documentation Requirements: • Active OT license + BLS CPR • Comfort with basic tech; email, texting, tablets, EMR systems (training provided) • Reliable transportation to see patients in their homes Job Type: Full-time Pay: $64.00 - $120.00 per hour Benefits: • Employee assistance program • Flexible schedule • Mileage reimbursement • Relocation assistance • Travel reimbursement Work Location: On the road
