Please note: This is a full-time, permanent, 1099 (contract-based) position Looking for Clinical Care Coordinator (Medical Assistant). • Place outbound calls to patients to review care plans and assist with coordination services. • Ensure medical histories are accurate and updated in the care coordination plan. • Educate patients about the care coordinator's role in collaboration with the primary care physician. • Monitor and report patient outcomes and provide chronic care education monthly. Job Type: Full-time Kindly Review the Complete Job Details Below • Pay: 1) Training Period (3 Days): ✔ Paid at $16 per hour (for training period) 2) After Training: ✔You will be provided a daily list of 20 existing patients, assigned to you for assessment calls. ✔ Complete at least 10 patient assessments per day ✔ Paid $10 per patient assessment ✔ If you complete more than 200 assessments in a month, you will earn $12 per patient for all assessments above 200. 3) Work Schedule & Minimum Earnings ✔ 5 days a week (Monday to Friday) ✔ minimum 10 assessments per day → 50 per week → 200 per month ✔ Minimum monthly earning: $2,000 (200 assessments × $10) ✔ Most team members comfortably complete 250 assessments per month, which also increases overall earnings. Schedule: Monday to Friday Location: Remote. Experience: Caregiving: Fresh to 1 Year License/Certification: CPR Certification or CNA or any prior experience in health care sector will be preferred.
