STATEMENT OF PURPOSE: The Lead Medical Coordinator is responsible for overseeing health services as well as coordinating the provision of timely, appropriate health services for children in care. They are expected to collaborate proactively with medical and dental providers, as well as the Department of Health (DoH). This includes facilitating referrals, coordinating appointments, monitoring follow-up care, and maintaining clear communication with providers and caregivers. The Lead Medical Coordinator supervises other Medical Coordinators and provides appropriate staff training on medical, dental, and public health topics relevant to pediatric, adolescent, and migrant health. They are responsible for developing and maintaining policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post-release health care planning/care coordination. They should also advocate for the children’s overall well-being by addressing preventive, acute, and ongoing health needs in alignment with best practices and regulatory requirements. EXPERIENCE: Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care, and be a Licensed Registered Nurse (RN) with a Bachelor’s, or a higher health-related qualification (e.g., Physician Assistant, Nurse Practitioner). DUTIES & RESPONSIBILITIES: All the responsibilities listed below are considered essential functions of this position. Additionally, this position is expected to ensure that the core operating values and the strategic direction of the agency are adhered to: 1. Conducts intake assessments through interviews with patients, case managers, and relatives to gather medical case history. 2. Create and contribute to patients’ medical records as needed. 3. Respond to inquiries, concerns, and complaints from patients, their parents, or guardians and make decisions to address concerns/complaints. 4. Available to attend all required meetings, provide training, etc., as required. (e.g.: CFTM) 5. Plans and provides nursing care for children. 6. Makes nursing diagnoses of the medical and emotional status of patients and recommends treatment options. 7. Designs appropriate treatment plans for clients. 8. Develops and provides training to staff, parents, and children as needed. 9. Consider the age of the patient and his or her emotional stability when determining appropriate treatment. 10. Administers and notes reactions to psychotropic drugs and other medications. 11. Arranges further medical attention when necessary. 12. Assists the client in understanding the nature of emotional disturbances and accepting the need for treatment. 13. Counsel patients and family members before, during, and after treatment. 14. Maintain a safe and secure environment for staff and patients. 15. Test for vital signs of pain and ensures treatments of pain. 16. Documents all patient information on medical charts and follows physician’s written orders. 17. Maintains patient confidentiality. 18. Maintains accurate records and documentation of patient treatments, immunizations, and laboratory results according to standard medical recordkeeping. 19. Performs administrative assignments such as filing, answering phones, typing, and completing progress notes in the computer system. 20. Track and maintain medical supply inventory along with other staff members. 21. Provides courteous, high-quality service to patients and members of the public by personally responding to requests for service or making appropriate referrals. 22. Provide guidance and supervision to medical staff. 23. Other duties as required by the day-to-day operation. EMPLOYMENT STANDARDS: Ability to: 1. Apply professional nursing care principles and procedures in the evaluation and treatment of patients. 2. Develop interpersonal therapeutic relationships with patients. 3. Keep accurate written charts and records of patient activities and progress. 4. Comply with laws, rules, regulations, protocols, and procedures. 5. Use professional judgment to ascertain and apply appropriate intervention techniques with patients in crisis. 6. Communicate effectively with a variety of individuals and function calmly in situations that require a high degree of sensitivity, tact, and diplomacy. 7. Provide prompt, efficient, and responsive service. 8. Exercise appropriate judgment in answering questions and releasing information; analyze and project consequences of decisions and/or recommendations. 9. Available to board flights with children during weekdays and weekends if advanced notice of at least 24 hours is given. 10. Demonstrate strong oral and written communication skills.
