Job Details Level: Experienced Job Location: Detroit, MI 48209 Position Type: Full Time Education Level: 4 Year Degree Job Shift: Day Job Category: Nonprofit - Social Services POSITION SUMMARY: The Integrated Duel Disorder Treatment (IDDT) and Assertive Community Treatment (ACT) Case Manager is responsible for providing comprehensive, integrated treatment to individuals with co-occurring serious mental illness (SMI) and substance use disorders (SUD) using the evidence-based Integrated IDDT/ACT model. This role focuses on stage-wise interventions, motivational interviewing, and person-centered, recovery-focused care delivered in both office and community-based settings. The IDDT/ACT Case Manager is primarily responsible for providing support and care management services to consumers who struggle with serious and persistent mental illness and whose symptoms result in serious impairments in several areas of their lives, including employment, social relationships, residential independence, money management and physical health and wellness. The IDDT and ACT Case Manager participates in a multi-disciplinary team model whose goals are to support clients to reduce hospitalizations and to restore and enhance the emotional well-being and level of social functioning of clients. As part of a comprehensive community mental health team, the IDDT/ACT Clinician/Therapist delivers crisis intervention, outreach and advocacy. Qualifications MINIMUM JOB REQUIREMENTS: Bachelor's degree in social work, or related field Minimum of 1 year of experience working in Mental Health or Social Services is required Experience working with community organizations as an advocate for high-need individuals preferred Familiarity with co-occurring disorders (SUD and MH) Current licensure to practice social work and related mental health services in the State of Michigan Bilingual fluency in Spanish preferred SALARY: $40,950 and up, based on experience MiSide is committed to a diverse and inclusive workplace.
