Full Description

The Per Diem Case Manager at MD Anderson Cancer Center integrates and coordinates care facilitation and discharge planning functions. This role directly impacts patient care, satisfaction, and institutional costs by ensuring seamless transitions and optimal outcomes across the continuum of care. The ideal candidate must have a Bachelor's degree in Nursing (BSN), hold a current RN license in the State of Texas, and have three years of professional nursing experience, to include experience in coordinating care for high risk and complex patients. The candidate must also maintain American Heart Association Basic Life Support (BLS) certification. Per Diem Flat Annual Rate: $51,376 or $52.00/hour. Based on a 19-hour work week. Essential Job Functions: • Assess individual patient needs based on practice models for high-risk or complex care issues • Coordinate, facilitate, and monitor care for patients throughout the continuum to ensure optimal clinical, functional, social, cognitive, satisfaction, and financial outcomes • Oversee discharge planning and facilitate transitions to community settings • Intervene, advocate, and facilitate care changes on the patient's behalf • Act as a liaison for multidisciplinary teams, external physicians, payors, and community agencies • Identify and implement strategies to improve processes, reduce costs, and enhance patient care and business outcomes • Other duties as assigned Education Required: Bachelor's degree in Nursing (BSN) Experience Required: Three years professional nursing experience to include experience in coordinating care for high risk and complex patients. Licensure/Certification Required: Current State of Texas Professional Nursing License (RN). American Heart Association Basic Life Support (BLS). Licensure/Certification Preferred: Certification as a Case Manager. Refer to the Position Description for specific ADA requirements and working conditions.
