LSSI is hiring a full-time Lurie’s Outreach Coordinator, Monday through Friday. Responsible for identifying, reaching out to, and engaging residents at Chicago area SNF and SMHRFs, providing education and information on the program and the scope of services available post-transition. The paid training you need to learn, grow, and succeed! Essential Functions: Engage Class Members and build rapport with staff at nursing home setting. Review chart at nursing facility and collaborate with providers, family, and guardians. Remain familiar with accreditation and licensure standards. Adhere to ethical standards of treatment according to NASW Code of Ethics. Ensure all documentation is accurately completed according to requirements and direction provided by the Illinois Department of Human Services Engage in individual and group supervision with supervisor. Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner. Position Qualifications: High school diploma or equivalent required. Background check clearance required. Billingual/Spanish preferred. Customer service experience preferred. Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing. Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
