Bachelor’s degree in human services, two years case management experience, bilingual Spanish/English preferred, valid driver’s license, and ability to work independently with diverse populations.
The Trafficking Victims Assistance Program (TVAP) Case Manager provides trauma-informed, person-centered case management and support services to qualified individuals. The position performs individual case management, conducts community outreach, and maintains collaborative relationships with referral resources. He/she is should be able to work independently in providing TVAP services to clients. Fully Remote position but must reside in Columbia, SC or surrounding areas. Essential Functions: 1. Perform new client intake. Duties will include the assessment of needs and personal goals and connecting clients to critical resources. 2. Providing a wide range of client services, including but not limited to: providing referrals to appropriate community services and care providers, assist with scheduling medical appointments, coordinating food, medical supplies, housing, childcare, transportation and accompanying clients to appointments as necessary. 3. Maintain proper documentation of all services provided by keeping detailed and clear notes of all activities with and on behalf of the client which includes tracking client needs and services, costs, expenditures, client communications and contacts. 4. Ensure follow up with service providers and Department of Social Services to secure benefits and support for clients. Education: Bachelor’s Degree in human service field. Experience: Two years of experience in case management. Preferred multi-cultural experience and experience in working with immigrants, refugees and/or anti-human trafficking effort. Specific skills/abilities: Knowledge of Case Management techniques. Ability to work independently. Computer skills (Microsoft Office Suite, navigating internet, etc. Driving is required. Must have valid driver’s license and clean driving record. Ability to work effectively with clients from diverse populations including foreign born clients, refugees, asylees, and survivors of human trafficking. Strong attention to detail and tracking important dates and information. Specialized knowledge, licenses, etc: 1. Bilingual (Spanish/English) candidates strongly preferred.
