Job Title: Registered Nurse Case Manager Job Overview: As RN Case Manager of Luminos Hospice, you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best-in-class hospice care. Responsibilities Coordinates and oversees all aspects of patients' home care. Collaborates with patient care teams to develop and implement care plans. Educates families on patient condition, needs and care. Performs head to toe physical assessments. Checks vital signs and manages medications. Administers and maintains catheters. Complete documentation timely and accurately. Supervises HHAs. Regularly communicates patient progress to the clinical manager, physician and care team. Other duties as assigned. Requirements RN license in the state of Texas Current driver’s license, reliable transportation, valid auto insurance Current CPR and BCLS certification Excellent verbal and written communication skills Two (2) years' experience as a registered nurse in a clinical care setting, hospice preferred Job Type: Full-time Benefits: Medical insurance through Blue Cross & Blue Shield Dental and vision insurance through Lincoln Financial HSA or FSA plans Disability insurance Company-paid basic life and voluntary life insurance Mileage reimbursement of 50 cents/mile 160 hours of PTO annually, plus 9 paid holidays Professional development opportunities
