Lifeworks is hiring a full time Assistant Case Manager to work with Individuals with ID/DD in one of our Residential Programs. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. Essential Duties: • Assists individuals served within the program obtain and maintain employment that is consistent with their employment goals. • Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills. • Provide direct care for ADLs and group activities; Lift and transfer individuals as appropriate to their needs. • Participate in scheduled supervision sessions with Assistant Director. Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams. • Participate in program-wide staff meetings and case review meetings as scheduled. Participate in team meetings regarding general program development, including curriculum development and schedules. • Attend in-service trainings as required. • Comply with all applicable State and Federal regulations, agency policy and procedures, and personnel practices. • Perform other duties as requested by supervisor(s). Requirements: • Experience working with Individuals with ID/DD • High School diploma or equivalent.
