Job Overview We are seeking a compassionate and dedicated Visiting Nurse to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring they receive the necessary medical attention and support. This role requires a strong understanding of patient needs, excellent communication skills, and the ability to work independently in various environments. Responsibilities • Conduct thorough assessments of patients' health conditions and develop individualized care plans. • Administer medications and treatments as prescribed by physicians. • Provide wound care, catheterization, and pain management to enhance patient comfort. • Monitor vital signs and report any changes in patients' conditions to the healthcare team. • Educate patients and their families on managing illnesses and promoting healthy lifestyles. • Collaborate with other healthcare professionals to coordinate comprehensive care for patients. • Maintain accurate documentation of patient interactions and progress in electronic health records (e.g., Cerner). • Support patients with disabilities by adapting care plans to meet their unique needs. Experience Candidates should possess the following skills and experience: • Proficiency in ICD-9 coding for accurate billing and documentation. • Experience in primary care settings, providing holistic patient-centered care. • Knowledge of critical care transport protocols and emergency room procedures. • Familiarity with hospice palliative medicine practices for end-of-life care. • Strong understanding of human anatomy to assess patient conditions effectively. • Previous experience working with individuals with disabilities is highly desirable.
