Position Summary: The Project REACH (Racial and Ethnic Approaches to Community Health) Coordinator is a full-time position. Under the supervision of the Director of Health Services, the incumbent: Program Planning and Implementation: Develop and implement strategies to address health disparities and promote health equity in targeted communities. Collaborate with community organizations, public health agencies, and stakeholders to design and execute culturally sensitive interventions. Community Engagement: Establish and maintain effective relationships with community leaders, organizations, and residents. Conduct outreach and engagement activities to ensure community involvement in program planning and implementation. Data Collection and Analysis: Oversee the collection and analysis of health data relevant to the targeted population. Utilize data to inform program strategies, identify health trends, and assess the impact of interventions. Collaboration and Networking: Work collaboratively with internal and external partners, including government agencies, non-profits, and community-based organizations. Participate in relevant coalitions and networks to enhance program effectiveness. Monitoring and Evaluation: Implement monitoring and evaluation processes to assess program effectiveness. Collect and analyze performance metrics to measure progress towards program goals and objectives. Communication and Reporting: Communicate program goals, activities, and achievements to stakeholders through regular reports and presentations. Prepare documentation for internal and external reporting requirements. Qualifications: Minimum Bachelor's degree in public health, community health, social work, or a related field. 1 year of experience in public health initiatives, program coordination, community health, health services, or related human service field is preferred. Knowledge of racial and ethnic health disparities, social determinants of health, and community engagement strategies. Experience and demonstrated competence working with commercial software applications, including all Microsoft Office applications and with the Internet. Excellent writing and oral communication skills. Bilingual proficiency (verbal and written fluency) in English and Korean is necessary. Knowledge of the Korean immigrant community is preferred. Valid driver’s license, access to the insured and registered vehicle, and willingness to use a vehicle to travel to outreach events or partner meetings.
