POSITION SUMMARY: The PRN AgeWell Cincinnati Concierge Care Manager, serves as a professional in the private-pay geriatric care management practice and is responsible for managing and providing Care Management services to clients and their family caregivers on an as-needed basis. This role is designed for experienced clinicians seeking a flexible schedule with the opportunity to build a caseload. There are no guaranteed hours or benefits. PRN Concierge Care Managers are assigned clients based on availability, skill set, and client acuity. MORE ABOUT THE ROLE: • Conducts comprehensive client assessments (medical, psychosocial, functional and environmental). • Develops individualized care plans in collaboration with clients and families. • Maintains client records. Completes and maintains all necessary documentation including clinical and billing documentation on a timely basis. • Maintains regular in person, phone and written contact/interaction with client, family/caregiver, and other parties to coordinate services and monitor client status. • Coordinate care across providers, community resources, and support services. • Provide guidance and advocacy during healthcare transitions and crises. • Facilitate family communication, updates, and care conferences. Provide education and coaching to clients and caregivers. • Provides a high level of customer service. Coordinates individual availability to clients and their families. • Serve as primary point of contact for assigned concierge clients and families. SCHEDULE & COMPENSATION: • PRN hours based upon caseload size • Flexible scheduling based on client needs and Care Manager availability • Community based/remote work depending on client needs • Expect to manage assigned cases independently within agreed-upon scope • Hourly compensation: $50 an hour • Paid as a W-2 employee • No benefit eligibility • Eligible for agency-required trainings (paid as applicable) POSITION QUALIFICATIONS: • Active, unrestricted professional license (LSW, LISW, or equivalent) • Bachelor’s degree required but Master’s degree preferred • Minimum of 2-5 years of Care Management or experience in geriatrics required. • Strong knowledge of healthcare systems, community resources, and care coordination. • Experience and demonstrated competence and comfort working with families in crisis. • Excellent interpersonal communication and organizational skills • Ability to work independently with minimal supervision • Valid Ohio driver’s license and proof of current automobile insurance Physical Requirements This position requires walking, standing, and sitting. Position requires the use of a car to make home visits to clients in Hamilton, Butler, Warren, and Clermont Counties.
