DSP (Direct Support Professional) Job description SUMMARY: The DSP will provide specific services to an emotionally, mentally and/or fragile challenged member based on the service plan created by the supervisor. As a DSP your responsibility is to help the members reach goals set in their service plans by providing assistance and interventions that encourage the member’s independence and growth, as well as enhance the family’s ability to support the needs of the member ROLE AND RESPONSIBILITIES Offer educational, advocacy, and support resources to child/youth and caregiver/family • Caregiver/Family Supports and Services can be provided in the home of the youth or a community setting. • Help teach the caregiver/family how to access resources independently and ensure appropriate services are received by their child • Address issues the caregiver/family is concerned about (as appropriate) in relation to supporting their child • Provide instruction to allow child/family to have a fully understanding of the child’s condition or illness • Educate and train the caregiver/family unit on resource availability so that they can better support and advocate for the needs of the child and appropriately access needed services. • Development and updating of service plans, including obtaining medical consenter signatures. • Obtaining consents and enrollment documents for newly enrolled children/youth. • Attend monthly waiver service provider meetings to stay current on practices and protocols. • Report incident(s) to supervisor upon discovery. • Maintain open communication with other service providers (e.g., Care Manager) to report on child/youth’s progress and identify any potential barriers and referrals. • Submit progress notes, service plans, expense logs, and service provider logs within set forth-required timeframes. Progress notes are due within 3 days of service provision so that approval from supervisor is completed by the 5th business day following service provision.
