Job Title: RN Home Health / Quality Assurance Location: New Lenox, IL Duration: 13 weeks of contract with possible extension. Shift Time: Day 8:00 AM - 4:30 PM (40hr Guarantee) Requirements: • 2yrs+ Home Health experience - REQUIRED • Previous Quality experience - REQUIRED • COS-C (Certificate for OASIS Specialist-Clinical) Certification - REQUIRED • Valid drivers license and car insurance - REQUIRED • Copy of agency's IL nursing license included in submission - REQUIRED • OASIS experience - Required • Manager is wanting to review local candidates! Profile Requirements: • Certifications (must be active at time of submission) • IL RN License (must be active at time of submission) • COVID card/status (Declinations will not be approved) Responsibilities • Implements and evaluates performance improvement, quality, and utilization activities. • Conducts reviews of medical records (plans of care, clinical and progress notes, etc.) for quality and compliance standards. • Facilitates interdisciplinary case conferences, monitoring the coordination of care and OASIS data. • Monitors OASIS data integrity and completes OASIS data submission. • Ensures the development, implementation, and update of the individualized plan of care. • Produces monthly reports to monitor quality of services delivered and publicly reportable data. • Assists with the evaluation of clinical competence and compliance. EMR: Matrix - will be provided an iPad. Territory: Within 15 miles (may increase if census is low and they need to offer the 40 hours) Census: 24-26 on case load. If given a bigger territory that number will go down. Mileage: Reimbursed at the standard IRS rate. Perform some Telehealth/Virtual visits in addition to home visits: Orientation: 2 days. First day will be in the office. Second day out in the field. Services: Catheter Care, Chronic Disease Management & Telemonitoring. Diabetes Management Home Health Aide Services.
