Benefits: • 401(k) matching • Health insurance • Paid time off Care Coordination Specialist – Wound Care Services Location: [Remote] Schedule: Monday–Friday, Flexible Hours Status: Full-Time Holidays: Closed on Major Holidays Job Summary: We are seeking a detail-oriented and organized Care Coordination Specialist to support our wound care team. This role involves managing incoming patient referrals, coordinating scheduling, communicating with providers and home health agencies, and ensuring smooth clinical operations. The ideal candidate will be a strong communicator and able to multitask in a fast-paced environment. Key Responsibilities: • Receive and process incoming patient referrals • Schedule patient appointments and coordinate with providers • Communicate with home health agencies to relay wound care orders and avoid scheduling conflicts • Liaise with PCP offices to obtain medical records and other documentation • Prepare and send out orders for DME and wound care supplies • Distribute and maintain daily schedules for clinical providers • Create and manage electronic consent forms; upload signed forms to patient charts • Provide administrative support to wound care providers as needed, including assistance with documentation, communication, and clerical tasks Requirements: • Strong organizational and communication skills • Ability to work independently and manage time effectively • Experience in healthcare coordination or administrative support preferred • Proficiency with electronic health record (EHR) systems is a plus • Understanding of HIPAA Privacy Rules and ability to maintain compliance Work Environment: • Monday–Friday with flexible hours based on workload, typical hours ranging from 7:00am to 5:00pm • Requires quiet, focus-friendly remote workspace • All operations closed on major holidays
