Bachelor’s degree, 7+ years in managed care or government program management, 3+ years in value-based care or population health, PMP or Lean Six Sigma preferred, deep Medicare and CMS Innovation knowledge, strong project management and analytics skills.
Description: • Serve as an organizational subject matter expert (SME) and tactical lead for ACO REACH and other Center for Medicare and Medicaid Innovations (CMMI) programs • Partner with colleagues from Market Operations, Finance, Clinical Programs, and other domains to develop and clarify business questions, think critically about solutions, and deploy cross-departmental efforts • Develop tactical plans to ensure consistent deployment of ACO REACH education and program implementation across markets • Lead program design efforts in population health data reporting and care delivery benefit enhancements across multiple external business partners • Lead development of specialist and SNF networks and serve as primary contact for ACO preferred providers • Identify opportunities and facilitate analyses to improve care delivery, support population health, and contextualize observed performance trends in Medicare REACH contracts • Work with teams across Honest to conduct ROI analyses of existing ACO REACH programs • Manage and facilitate periodic reports, as well as any necessary ad hoc reports, to provide insight into REACH performance • Partner with teams across the organization to ensure programs meet regulatory requirements, are implemented according to plan, and trackable to provide insights into progress and program efficacy • Maintain the value-based care program, including provider roster management, government program database entry, and tactical performance • Support preparation of presentations, updates, and strategy briefs for senior leadership, government agencies, and external partners • Assist the advocacy team in shaping responses to proposed rules or initiatives and translating operational insights into policy recommendations • Perform other related responsibilities as assigned.
