Overview Join our dynamic team as a Service Coordinator, where you will play a vital role in coordinating and managing services for elders, individuals with developmental disabilities, disable adults and children with special needs. As a Service Coordinator, you will be the central point of contact for clients, families, and service providers, driving positive outcomes through proactive management and compassionate support. This paid position is ideal for someone passionate about social work, team worker, and working closely with diverse communities to promote independence and well-being. Responsibilities • Coordinate and oversee individualized service plans for participants. • Build strong relationships with clients, families, service providers, and community partners to facilitate collaboration and trust • Manage administrative tasks such as maintaining accurate client records, documentation, and progress reports • Supervise team members involved in care delivery to ensure quality standards are upheld • Skills • Proven management and program development experience in social services or related fields • Strong ability to work effectively with people with developmental disabilities, demonstrating empathy and understanding • Excellent relationship management skills to foster trust among clients, families, staff, and community partners • Solid administrative experience with record keeping, documentation, and organizational skills This role offers an exciting opportunity to lead impactful programs that empower individuals with special needs.
