Requires a bachelor's degree in social or health sciences and two years of related care management experience, with skills in communication, organization, and teamwork.
GENERAL SUMMARY: Various Locations : Detroit Northwestern High School- Detroit Michigan University Prep Art & Design-Detroit Michigan • Van Dyke Lincoln High School- Warren, Michigan • The Care Manager serves as a member of the integrated, multidisciplinary care team for the Pediatrics School-Based and Community Health Program. • Functions as a liaison between the medical staff and the primary behavioral health provider to provide, plan, evaluate and monitor services from a variety of resources on behalf of and in collaboration with patients and/or their families. • Assists with connecting patients and/or their families to needed resources and provides ongoing care management of recommended programs/services. • Provides initial monitoring of care, focusing on whether the patients/families have been connected with the referral services. • Monitors the processes and outcomes with regular follow-up. EDUCATION/EXPERIENCE REQUIRED: • Bachelor's degree in Social Work, Community Health, Healthcare Services, Child & Family Services, Criminal Justice, Public Health, Psychology, Sociology, or Youth and Community Development. • Two (2) years of related experience in case/care management, including social services and intakes. • Ability to take a basic history including presenting symptoms, background information, etc. • Ability to co-facilitate groups with other providers. • Good verbal and written communication skills. • Good organizational skills including ability to prioritize, multi-task and meet deadlines. • Good interpersonal skills, including ability to develop and maintain trusting relationships, and work effectively with program staff and personnel from other agencies/partners. • Ability to convey social work practice, knowledge, goals, and values consistent with the Professional Code of Ethics. • Ability to be flexible with work schedule. • Ability to travel locally to visit service providers, SBCHP sites and patient's home or identified meeting site. Preferred skills • Bachelor's degree in: Social Work, Community Health, Healthcare Services, Child & Family Services, Criminal Justice, Public Health, Psychology, Sociology, or Youth & Community Development. • Two years' experience case/care management, working with youth, adolescents, and young adults, on a multidisciplinary/integrated team; either in an urban school environment, outpatient behavioral health and/or primary care, or a community youth agency with high-risk clients/families. • Skill set to work well as part of an integrated team, with excellent organization/written skills, including ability to develop and maintain trusting relationships, and work effectively with program staff and personnel from other agencies/partners. • Perform a wide variety of professional behavioral/social health services; to effectively communicate with patients, families, and other personnel. • Experience coordinating new programs and co-facilitating educational/prevention groups with school age students.
